00:00Becca Chilczenkowski: Welcome back to The Health Revival Show. We have Dr. Bill Rawls today — a physician, bestselling author, and thought leader in holistic health and herbal therapies. We're talking about cellular health. A lot of people talk root-cause medicine and bring up all the different things that bucket into that, but cellular medicine is ultimately where we want to go. That's the true root cause.
00:39Dr. Rawls: Forty years this past year since I graduated medical school. It's been quite a journey — and it hasn't been doing the same thing the whole time. I started medical school in 1985. I was drawn to obstetrics and gynecology because it was about bringing life into the world. I wasn't attracted to managing chronic illness at all, because I never saw people getting well.
But forty years ago, practicing OB/GYN meant practicing in a small group. I had two partners, sometimes one — which meant being on call every second to third night, every second to third weekend, Friday morning until Monday morning. And I'm one of those people who doesn't sleep if somebody's in labor. So I was sleep-deprived most of the time. Highly stressed, trying to be a father, a community person, a doctor, all of it.
After twenty years of that, I crashed. And it wasn't just not sleeping and then sleeping again and everything going back to normal. My whole body started falling apart, and nobody could put it back together. I did all the conventional testing. Well, your thyroid's a little bit off. This is a little bit off. So you take the thyroid medicine and it doesn't help.
03:10 Through it all, I kept asking different questions than my conventional colleagues were asking. One of them was: could this be a microbe holding my illness in place? Because even after I stopped obstetrics and had the time to sleep again, I couldn't sleep. I'd lost the ability. I had bad arthritis, heart issues, neurological issues. My whole body was in bad shape.
Eventually I got a positive test, like many people with chronic conditions that don't have another diagnosis do. I thought: antibiotics, I'll take antibiotics and get well. Several rounds of antibiotics made me sicker. So I lost confidence in the conventional system and started exploring. I became certified in holistic medicine and started a wellness practice — this was really before functional medicine was a thing.
04:40 By default and desperation, I turned to an herbal protocol, with low expectations. I was biased — I thought herbs were just weak versions of drugs. And I started getting my health back. I assumed that if these herbs were killing bacteria, they'd wreck my gut like everything else had. My gut got better. It was already a wreck, and it got better and better. Over about a five-year period, all of the symptoms gradually melted away.
It changed my career direction, and I've been doing it differently ever since. I don't see chronic Lyme the way other providers do, and I see a lot of commonality of this microbe factor with virtually every chronic illness. I don't see disease as a diagnosis. When I'm working with someone, I want to know why they're sick, not how they're sick. I'm sixty-eight, and I've had some of the best health of my entire life over the past decade.
Why Chronic Infection Testing Keeps Coming Back Negative
06:35Liz Roman: You shifted so much — careers, beliefs, continuing to dig and ask why. The proof is in the pudding: you've got your life back. So where did you land when you finally got that positive? And what did you find most beneficial? For the audience — this is not medical advice, and he's not giving you a prescription to go start oil of oregano or colloidal silver here.
07:45Dr. Rawls: The herbs are definitely at the top of the list. But testing — I don't put a lot of reliance on testing. We live in a society of test and treat and test again to see if the treatment is working. That works really well for medical therapies in certain situations, but it's mostly best for acute intervention. If somebody has an aggressive acute pneumonia and a specific antibiotic is the solution, testing becomes really valuable.
But with what we call chronic Lyme disease, the testing just isn't reliable — because most of the people being tested have a chronic condition, not an acute one. And it has to do with the nature of the bacteria.
09:24 That acute pneumonia is an aggressive pneumococcal bacteria doubling every 20 minutes. It grows very, very rapidly. It's a single microbe, it's really aggressive, and antibiotics work best when something is growing very fast — they depend on that rapid turnover. You can get that person well in two or three days, a week at most, before the antibiotic starts catching up with them and doing damage.
09:58 Borrelia, the Lyme bacteria, has a doubling time of 12 to 24 hours. Pneumococcal pneumonia, 20 minutes. Borrelia, 12 to 24 hours. It grows unbelievably slowly — more slowly than the flora in your gut. That makes it less of a target for the immune system and much less susceptible to antibiotics. So antibiotics by default just don't work very well for it.
How Microbes Go Dormant Inside Your Cells
10:33Dr. Rawls: The mission of that microbe is different. Pneumococcal pneumonia is a microbe trying to overwhelm the host. All Borrelia wants is to have a presence in the host body. So the two mechanisms it uses are growing really slowly and invading cells. When it gets in the body, it's trying to outrun the immune system as fast as it can and penetrate tissues and cells. Brain, heart, joints, everywhere it can get. And inside the cell, it has refuge.
If your immune system isn't healthy, it might attack your tissues immediately. But if your immune system is healthy, it takes a deep dive into your tissues and goes quiet. It can invade cells and go dormant inside them. By doing that, it creates a reservoir that can persist for a very long time.
11:50 I probably picked the bacteria up when I was a kid. That's what people don't realize — there's this impenetrable reservoir that may be totally quiet. This idea that you get bitten by a tick carrying the Lyme bacteria, you get sick, and you need antibiotics is really unusual. Most people don't get sick, or hardly get any symptoms at all. It's not until they're stressed later in life — like taking night call for twenty years and not sleeping — that the body becomes stressed, these things reactivate, and they start breaking down our tissues. That's when disease happens.
12:32 There are twenty-one species of Borrelia that we know of, and maybe many more we don't. That strategy of slow growth, invading cells, going dormant — it's the same strategy used by Babesia and Bartonella, and Mycoplasma and Rickettsia. And beyond that: when you were a child, you were exposed to mycoplasmas and chlamydias and Epstein-Barr, and all of these things operate on the same formula because it's super effective. They don't care about overwhelming you. All they want is a presence in your body.
So when something is chronic and sometimes low-grade, testing isn't that good — it's designed for acute infection. When someone has these complex chronic illnesses, you assume they have reactivation of these things. Might be Borrelia, might be Bartonella, might be a mycoplasma. My list is well over a hundred microbes long, and I think we're just scratching the surface. We all have some of this. It's not until we become vulnerable that we become chronically ill.
Perimenopause and the Boiling Point
14:22Becca Chilczenkowski: We work a lot in the perimenopause age range, and all of these women reach this point where symptoms start popping up and they say, "I never dealt with any of this." You might not have been dealing with symptoms, but there were likely things going on in the body that weren't addressed — and the level of stress you reach because of hormone fluctuations and immune triggering in perimenopause, now it shows up. We categorize and label it as an EBV problem or a Lyme problem, when in reality it's a cellular problem and a stress problem. People want the diagnosis. They want the answer, when maybe that's not the whole picture of how we're going to resolve it.
15:54Dr. Rawls: That's right. Our conventional system does a very good job with acute intervention — a tubal pregnancy, an acute pneumonia, a clot in the heart, a broken leg. We're great at that. But we have a flawed idea of what chronic illness is and how to manage it. We don't look at it at a cause. We look at the manifestations, and we treat the manifestations, and that's what drugs do. So people aren't getting well.
16:51 I'll introduce a term I use. That point you're talking about, I call the boiling point. You reach a point where you have enough microbe reactivation continuing, fueling that cellular damage, that weakened cells fuel more microbe reactivation — and it becomes a vicious cycle. Once you're in that, it's self-sustaining. And a lot of times, just changing your health habits, or doing the kinds of things that might have reversed it early, aren't going to reverse it after that point.
17:44 It all adds up. You have all these stresses in your life, you're not eating a great diet, and then perimenopause comes along and throws a monkey wrench in everything, and that tips the balance. Then you have a reactivation of Epstein-Barr or mycoplasmas or whatever else you've got in your system, and you're in trouble. And it's a kind of trouble the conventional system can throw drugs at to block inflammation or symptoms, but it's not going to fix you.
Why the Diagnosis You're Chasing Might Not Matter
18:23Dr. Rawls: Complex chronic illness is a better comprehensive term than trying to do the individual things. To me, the only difference between fibromyalgia and chronic Lyme is that a person with fibromyalgia hasn't gotten a test for Borrelia yet. They may or may not have it — but when you look at how we solve this problem, knowing exactly which microbe isn't as necessary, because we're not talking about microbes that can kill people like that aggressive pneumonia. We're talking about a spectrum. And even when you do testing, you typically aren't going to find everything that may be active.
19:15Liz Roman: It depends on the test. I say this about stool tests — labs and symptoms could be very glaring with a parasitic infection, but we don't catch it on a single stool test. They're sneaky. They have different life cycles. You don't even know if it's only invading the GI tract; it can be invading the liver, blood, brain. And candida can harbor up to 79 other toxins in the body. What you're saying is very in alignment with what we do in our practice — being broad spectrum, following symptoms more than labs, helping the immune system come back to balance and move between TH1 and TH2 with resilience. Our environment is going to forever have these microbes. Chasing an individual microbe leaves the terrain still susceptible to all the others.
Mold: Why Only Some People in the Same Building Get Sick
20:50Liz Roman: I have a case right now. She was doing amazing — she came in going to the bathroom up to 20 times a day, tolerating three or four foods, no ileocecal valve, because doctors took it out and told her she'd never have to worry about any issues. Over time she's developed a strong reaction and infection to mold mycotoxins in the school where she works. Conventional medicine is disregarding the reports that came back, and they want her to see immunology specialists — and what's their answer going to be? More antifungals, more antibiotics. How do you help clients when they can't get out of the environment? In this case it's her job. That's her income for her family.
22:00Dr. Rawls: So she's trapped in that environment. But there are other people trapped in that environment who aren't sick, right? There are a couple of different reasons for that. You really have to ask: why are molds producing mycotoxins in the first place, what do they actually do, and why are only some people affected?
When you have mold contamination in an environment, you have two factors. One is mold spores, released by the mold. The other is the actual mycotoxins in the air. What the mold produces mycotoxins for is to kill bacteria — bacteria are mold's arch enemy. Mycotoxins are produced to damage the cells of bacteria, but in the process they damage any cells.
Mold spores are how they spread. We can have an allergic reaction to mold; sometimes they take root in sinus cavities, but they don't necessarily enter the body.
23:22 One thing I'd say is that when people are sensitive to mycotoxins, they're vulnerable. Their cellular health is vulnerable. We are all exposed to mold and mycotoxins all the time — mold is pretty much ubiquitous in any environment at some level, and we get it in our food in various ways. When we breathe in mycotoxins, they're absorbed into our system and into our bloodstream, and they damage our cells. If our cells are already vulnerable, if we already have cellular stress, then that adds to the misery. It intensifies the cellular stress and moves it closer to that reactivation point — that boiling point where you have reactivation of other things in the body.
What to Do If You Can't Leave a Moldy Environment
24:27Dr. Rawls: Quick story. After I quit doing OB/GYN, I was asked to leave my office and started a medical office in a small town on the coast of North Carolina. There weren't any office buildings, so I bought this fifty-year-old building and moved in with optimism — and learned about mold the hard way. The crawl space was completely saturated with mold. The wallboard had mold in it. It was the worst environment for me to move into.
I was in that building for ten years. I recovered my health completely within that building. I couldn't afford mold remediation. That was a case of learning to live around it. I used ozone generators on the weekends. I used air filters in every single room. I got out of the office as much as I could during the day to go on walks. And I started intensely changing my lifestyle, my diet, taking herbs at very high levels. Over that five-year period — that's probably one of the reasons my recovery was slower than it would have been, because my recommendation for mold is to get away from it as fast as you can. But I was trapped. I didn't have a choice, and a lot of people are in that.
So in that kind of case, you do everything you can to reduce exposure. If that person has to buy air filters to put in her classroom to reduce the circulation of spores and mycotoxins as much as possible, she needs to do that — whether the school covers the expense or not. And that idea of restoring cellular health: be particular about your diet, your sleep, and your environment everywhere outside that location.
28:01Liz Roman: Lower the load, lessen the burden — couldn't agree more. Get non-toxic wipes for the room, get an air purifier in the classroom or office. I even have clients who move one unit with them: bedroom to office and back home.
The Five Things That Stress Every Cell in Your Body
29:22Dr. Rawls: I do a phased program — we start with restoring vagal tone, then move into detoxification and gut recovery, and finally getting a true handle on cellular health. But here's a different way of thinking about health in general.
Your body is 90% cellular, held together by a network of connective tissue. Absolutely everything that happens in your body is done by cells — the heart beating, brain impulses firing, thyroid hormone being produced. Every cell is producing messengers, and some cells, like thyroid and adrenal cells, specialize in coordinating cellular function. When you want to simplify a problem down to its smallest functional unit, for the body that's a living cell.
All cells have structural similarities. Think of them as little biological machines with very specific operating requirements. If you meet those requirements, the cell performs just like it's supposed to. Flip that around and ask, what if they don't get that? Those are the stress factors. And there are only five categories of things that stress cells. That's it.
32:00 One — nutritional stress. A high-carbohydrate processed-food diet gives our cells all the wrong nutrients to thrive. A whole-food diet rich in vegetables and lean protein is what our cells need to function. Vegetables are really important for the GI tract and everything else too.
32:41 Two — environmental stress. Mycotoxins are a factor, no doubt — our tightly closed buildings with moisture inside are great environments to grow mold. But the big toxic problem nobody's paying attention to is chemicals from fossil fuel combustion and the chemical industry. We have eight billion people on the planet and two billion cars. Fossil fuels came from natural organic plant matter, but those chemicals were distorted by millions of years of pressure in the earth, so they're no longer compatible with life. Nobody would drink gasoline and expect to survive it. These unnatural hydrocarbon chemicals penetrate our cells very easily, and when they do, they damage the way our cells work — our cells try to pull them in and use them, and they don't behave like natural chemicals. It disrupts messaging. If our cells can't communicate, they can't function.
34:25 Three — chronic mental stress. Cells need downtime to recover, using the process we call autophagy — cells are constantly rebuilding, and a big part of that happens at night. Daily stress has to be balanced by recovery time. The average American sleeps six and a half hours a night. You need a full eight, and if you don't get it, you start your day with cells that haven't recovered from the day before. Do that over and over and it'll catch up with you.
35:06 Four — physical stress. Trauma, excessive cold, excessive heat can all damage cells. But being sedentary takes away something critical: blood flow. Cells need blood flow to deliver nutrients and purge waste. When you don't move, all the spaces between the cells get congested and blocked, and cells can't purge toxic substances. So the first step in detoxification is increasing blood flow — because the toxins are in our cells. That's where the damage is done. The cells are trying to get rid of them, but if the spaces between the cells are congested, they can't. Whether you're doing it with exercise or sauna, blood flow is critical for recovery.
36:03 Five — the microbe factor. Microbes are the cell's arch enemy, and we're being hit all the time. It's not just microbes in our gut or on our skin — we have things in our tissues just waiting for an opportunity.
So: eating a good diet, sleeping well enough, cleaning up your air and water and food, getting out and moving during the day, and taking something for extra protection against that microbe factor — that's a formula for wellness right there. Once somebody is past that boiling point, though, their body is in fight or flight, their gut is shut down, their detoxification pathways are blocked. You have to systematically unpack that.
Why You Can't Sleep When You're Exhausted
37:21Becca Chilczenkowski: You mentioned beginning with vagal nerve toning and supporting the gut-brain axis. What are your favorite ways to stimulate and support better function of the vagus nerve?
37:43Dr. Rawls: Why did I lose the ability to sleep? When cells are stressed — when cells are injured, like if you injure your finger — cells release chemicals that activate nerves that tell the brain something is wrong. Well, if your whole body is in distress, twenty-four/seven every cell in your body is sending distress signals to your brain constantly, and it's overwhelming.
The answer is that you have to solve the cellular stress so that doesn't happen anymore — but that takes time. So you really have to override it so the body can settle down and get into a healing mode. That fight-or-flight response basically shuts down everything. It puts all your cells on high alert. It activates your brain with adrenaline, so you can't sleep. It shuts down your gut. It shuts down detoxification pathways. So it really has to come first.
38:49 One thing we use is an herbal preparation with ashwagandha, some calming herbs, and theanine, just because it helps take the edge off. A lot of people with chronic illness end up habituated to benzodiazepines and sleeping pills, which can be a problem. I did. I was desperate at one point, and I can tell you that's not a good way to go if you can avoid it.
Then there are practices. Meditation, brain retraining. For me, a lot of yoga and qigong with breathing exercises. What you're doing is teaching your body to ignore all those distress signals blazing from your cells, so your body can settle down and your adrenals can settle down — so you move closer to a place where the body can actually heal. It's teaching your body how to be calm again.
Do Vagus Nerve Devices Actually Work?
40:20Dr. Rawls: One thing I've been interested in but haven't explored at a high level is vagal nerve stimulators, which would be fantastic if they actually worked. I haven't found great evidence of that yet.
40:40Becca Chilczenkowski: There are a couple of devices we use. The Zenbud is one we've been using more recently — it triggers the spot in the ear, with vibration. The research on vibration stimulation of the vagus nerve is technically good, but in application, how much is it actually helping these people?
41:10Liz Roman: I get asked about these a lot on the gut side, because I do a lot with chronic constipation and motility. The Pulsetto isn't my favorite — it's sending a lot of electrical pulses, and people feel tingling or buzz. The Zenbud uses ultrasound waves, which has been shown in research for neuromodulation and vagal toning. It helps with stress relief and deeper parasympathetic activation. But my thought on many of these things, like lymphatic drainage: you can do everything for free if you truly believe — and buying a product isn't going to help you believe.
42:00Dr. Rawls: I like shortcuts. I tried EEG devices — there was a device called a Muse that gave you feedback. They weren't that great. They ended up in the closet.
42:25 The things I do for myself matter the most. Qigong was something I really got into, and for twenty years now I've done yoga classes three or four times a week — start my day with it. Those things are so powerful. And just getting out for a walk. For people who have the capacity, going for a walk can be extraordinary. Herbs are my go-to; they've helped by far more than anything else I've done.
Lyme vs. EBV vs. Mold vs. SIBO — How to Tell
43:10Liz Roman: A lot of complex chronic cases have very similar symptoms depending on the microbe. There are a few distinct things about Lyme compared to EBV, or mold mycotoxin compared to SIBO — but at the end of the day we still have to restore the terrain, support our cells and the signaling, and bring back resilience. For someone listening who's thinking, "Okay, he mentioned Lyme, Bartonella, Borrelia" — what are the signs that point you in that direction?
44:25Dr. Rawls: If you understand chronic Lyme, you really understand everything. When you look at any kind of symptom, it's always a reflection of cellular stress. Our body is telling us exactly what's wrong — whether it's a heart symptom or a muscle symptom, it's cellular stress in those areas.
Some clues: we know certain microbes invade certain cells. Borrelia invades chondrocytes — cartilage — plus heart cells and nerve cells. Bartonella invades blood vessels, which is quite different. People with fatigue tend to be more viral than bacterial; bacterial tends to cause pain. But it really depends on which cells the microbes are emerging from and invading.
45:30 There are so many lines that cross. Borrelia invades the cells surrounding our nerves — oligodendrocytes — and scavenges myelin. That's what causes a lot of the neurological symptoms. Well, that's exactly what's happening in multiple sclerosis, and most people with MS test positive for Borrelia. So is MS Lyme disease? No — it can be caused by chlamydia and other microbes too, and there are genetic factors that might predispose someone to end up with MS instead of Lyme with neurological symptoms.
When you look at chronic illnesses, the margins blur. So I like to look at it from the point of view of what cells are being stressed and what's driving those forces — always with a microbe factor, not necessarily always knowing which. Maybe in fifty years we'll have testing that tells us exactly what's in our tissues. I think we're a long way off from that.
46:50 That's the nice thing about herbs. As opposed to an antibiotic, which is a single agent, you're talking about thousands of chemicals in an herb — you're borrowing the plant's defense system to affect these variables inside our body. That's built for a wide range of viruses, bacteria, protozoa, yeast. It affects parasites. But it's also protecting our cells, and that's what's unique about herbal therapies: they're doing a lot more than just killing bacteria.

- Borrelia
- The bacterium behind Lyme disease. Doubles every 12 to 24 hours and invades cartilage, heart, and nerve cells. Twenty-one known species.
- Bartonella
- A tick-borne bacterium that preferentially invades blood vessels, producing a different symptom picture from Borrelia.
- Mycotoxins
- Chemicals mold produces to kill competing bacteria. They damage any cell they contact, including human cells.
- Mold spores
