FitMom Legal Disclosures & Client Consents

This document supplements FitMom’s Terms & Conditions and Privacy Policy and applies to all clients, customers, participants, and users of FitMom services.

By enrolling in, purchasing, or participating in any FitMom program, service, or community, you acknowledge and agree to the disclosures and consents below.


1. Scope of Practice Disclosure

FitMom Lifestyle, doing business as FitMom, provides functional health coaching, education, and wellness support.

Liz Roman and Becca Chilczenkowski are board-certified functional health practitioners, but they are not medical doctors, physicians, or licensed healthcare providers. Other FitMom practitioners operate under the same functional coaching and educational scope.

FitMom practitioners:

  • Do not diagnose medical conditions

  • Do not prescribe medications

  • Do not provide medical treatment

  • Do not replace your primary care provider or specialist

All services are intended to support education, awareness, and lifestyle-based decision-making.

You acknowledge that any recommendations provided are non-medical, non-diagnostic, and educational in nature, and you agree to consult your physician or licensed healthcare provider for medical advice, diagnosis, or treatment.


2. Telehealth-Style Consent (Non-Telehealth)

FitMom services may include remote coaching sessions, video calls, messaging, lab education, and digital communication.

You understand and agree that:

  • FitMom does not provide telemedicine or telehealth services

  • Communications are for coaching and education only

  • Services are not intended for medical emergencies

  • You should seek immediate medical attention for urgent or emergency health concerns

You consent to receiving services electronically and acknowledge the inherent risks of remote communication, including potential technical failures or unauthorized access beyond FitMom’s control.


3. Client Responsibility Acknowledgement

You acknowledge and agree that:

  • You are voluntarily participating in FitMom services

  • You are solely responsible for your health decisions and actions

  • You will consult with your healthcare provider before implementing changes

  • You assume all risks associated with participation

FitMom does not guarantee outcomes or results.


4. Checkout & Payment Legal Acknowledgement

By completing checkout, enrolling in a program, or submitting payment, you expressly acknowledge and agree that:

  • You have reviewed FitMom’s Terms & Conditions and Privacy Policy

  • All sales are final and non-refundable

  • Payment plans represent a commitment to pay the full program fee

  • Failure to participate or complete the program does not release you from payment obligations

  • You authorize FitMom to charge your selected payment method

You agree not to initiate chargebacks or payment disputes without first contacting FitMom support.

Unauthorized chargebacks may result in removal from programs, termination of access, and collection efforts.


5. Platform & Communication Consent

You consent to communication and service delivery through third-party platforms including, but not limited to:

  • Practice Better

  • Skool

  • Stripe

  • ManyChat

  • HubSpot

You acknowledge that FitMom is not responsible for platform outages, delays, or third-party data handling practices.


6. Electronic Signature & Acceptance

By checking a box, clicking "I Agree," submitting payment, or participating in any FitMom service, you affirm that:

  • You have read and understood this document

  • You voluntarily agree to all disclosures and consents

  • You understand this agreement is legally binding
